Welcome to Truman Athletics

All athletes new to Truman (freshmen and transfers) must complete the following 6 steps prior to any athletic participation at Truman. Complete all steps through the Athletic Trainer System (ATS).

Step 1 — Create a Profile in ATS

Create your profile in the Athletic Trainer System (ATS) (opens in new tab) using the credentials below.

Initial Login Credentials
Athlete ID: new (type the word “new”)
Password: new (type the word “new”)
Database: atstruman

Once you open the form, change your ID to match your Truman ID number (starts with two zeros) and select your own password.

Step 2 — Upload Sickle Cell Results

All NCAA Division II athletes must provide results of a sickle cell solubility test (SST) prior to any athletic participation. All children born in the United States were tested for sickle cell trait at birth.

  1. Click on the eFiles tab.
  2. Click Upload Document.
  3. In the description box, enter your name.
  4. Select Sickle Cell Result from the Document Type dropdown menu.
  5. Select the file containing a picture or PDF of your results.
  6. Click Upload.

Step 3 — Complete Insurance Information

Every athlete must have their own personal insurance policy in order to practice or participate in any athletic activity at Truman. The questionnaire will ask you to upload a clear picture of your insurance card.

Have a digital copy of your insurance card ready before you begin. You will need to upload it during this step.

Step 4 — Complete Emergency Contact Information

This emergency contact will only be used in case of a true emergency. Please provide information for at least 1 contact.

Once your emergency contact is submitted, you will be brought to a new page. To complete the next steps, select which form you would like to fill out from the dropdown box.

Step 5 — Complete the Pre-Participation Physical Form

  1. Click on the Forms tab.
  2. Choose Truman Pre-Participation Physical Exam from the dropdown menu.
  3. Click New.
  4. Complete the form accurately and completely, then click Save.
About the Physical Exam
The Truman team physician and athletic training staff will administer a physical exam to each student-athlete. The physical exam form guides the medical staff in performing the best exam. Answer all questions accurately and completely.

Step 6 — Complete the Concussion Acknowledgement Form

  1. Click on the Forms tab.
  2. Under Form Name, select Concussion Acknowledgement from the dropdown menu.
  3. Click New.
  4. Read the NCAA Concussion Fact Sheet.
  5. Complete and sign the form, click Sign, then click Save.
Why This Matters
Every year all Truman athletes must be informed about the risk of concussion and the need to report concussion symptoms. Being informed is one part of Truman’s commitment to protecting the brain health of its student-athletes.
