All athletes new to Truman (freshmen and transfers) must complete the following 5 steps prior to any athletic participation at Truman.
 If you have difficulty completing these steps, try this video.
  1. Create a profile on the Athletic Trainer System (ATS)
    Athlete ID: new  (type the word "new" as your Athlete ID)
    Password: new (type the word "new" as the password)
    Database: atstruman
    ***Once you open the form, change ID to match your Truman ID number  (starts with 00) and select your own password***
     
    2. Upload Sickle Cell Results
  • Click on the eFiles Tab 
  • Click "Upload Document"
  • In the description box, enter your name
  • Select "Sickle Cell Result" from the Document Type drop down menu
  • Select the file that contains a picture or pdf of your results
  • Click Upload  
    All NCAA Division II athletes must provide results of a sickle cell solubility test (SST) prior to any athletic participation. All children born in the United States were tested for sickle cell trait at birth. Click here to learn more about getting a copy of those results.
     
    3.  Complete the Insurance Information on the Insurance Tab
    Every athlete must have their own personal insurance policy in order to practice and or participate in any athletic activity at Truman. Read about Truman’s insurance requirements and policy on the Medical Information Tab on Truman’s athletic website. The questionnaire will ask you to upload a clear picture of your insurance card. (Have a digital copy of your card ready.)
     
    4. Complete the Emergency Contact Information
    This emergency contact will only be used in case of a true emergency. Please provide information for at least 1 contact. Once emergency contact is submitted, you will be brought to a new page. To complete the next steps, select which form you would like to fill out in the drop down box.
     
    5. Complete the Pre-Participation Physical form
    a. Click on the Forms tab
    b. Choose Truman Pre-Participation Physical Exam from the drop down menu.
    c. Click “New”
    d. Complete the form accurately and completely then click Save

    The Truman team physician and athletic training staff will administer a physical exam to each student athlete. The physical exam form will be used to guide the medical staff in performing the best exam. Answer all questions accurately and completely.
     

    6. Complete the Concussion Acknowledgement form
    a. Click on the Forms tab
    b. Under Form Name, select Concussion Acknowledgement from the dropdown menu
    c. Click “New”
    d. Read the NCAA Concussion Fact sheet
    e. Complete and sign the form, click “Sign”, then click Save

    Every year all Truman athletes must be informed about the risk of concussion and the need to report concussion symptoms.  Being informed about concussion risk and concussion symptoms is one part of Truman’s commitment to protecting the brain health of its student athletes.
