Freshman/Transfer Bulldogs

Welcome to Truman!  Before you begin your career as a Bulldog, there is some medical information you will need to supply.  Please read the following information carefully.
 

Medical Paperwork:

Every new Bulldog must create an electronic medical file. This file must be complete before you can begin any athletic activities at Truman.  Click here for directions on completing this paperwork.  A complete medical file must contain the following:

  • Emergency Contact form

  • Pre-Participation Physical form

  • Sickle Cell blood test results

  • Concussion Acknowledgment form
  • Insurance information (including uploading a picture of your insurance card)

Sickle Cell:

 All NCAA student-athletes are required to provide results of a blood test for sickle cell trait.  These test results must be submitted to the athletic training department prior to any athletic participation.  All babies born in the United States are tested for sickle cell at birth.  These test results may be requested from your primary care physician.  Learn more about how to obtain your newborn sickle cell results.  If you cannot obtain a copy of these newborn screenings, you must request a sickle cell solubility test from your physician and supply a copy of those test results prior to participation in any athletic activity.  You will be asked to upload a copy of these results to your electronic medical file.

Insurance:

All Truman athletes must have their own insurance policy.  You will not be allowed to participate in athletics until you have proof of insurance coverage.  The cost of health care very high.  In order to defray some of the cost, Truman requires all student-athletes to be insured under a primary insurance policy.  No student-athlete will be allowed to practice until they provide proof of primary insurance.  Click here to learn more about purchasing personal health insurance.


The Truman State University athletic department carries a secondary athletic insurance policy for all Truman Varsity athletes. This policy covers injuries that occur while participating in official practice activities.  Injuries and illnesses that occur outside of formal athletic practices and competition are not covered by this policy.    Click here to learn more about Truman’s secondary health insurance.

Physicals:

As a new athlete to Truman State University, you are required to have a physical exam before participation in Bulldog Athletics. In order to allow the Truman medical staff to understand your health and to ensure a thorough physical, this exam must be completed by the Truman team physicians (a high school physical or physical from a personal physician will not be accepted).  The Truman physical is at no cost to the athlete.  The physicals are scheduled for the beginning of the season and coordinated with the team schedule. 

The medical history portion of the exam is extremely important.  Please look at the medical conditions listed below.  If you have experienced any of these conditions, come prepared with paperwork that documents the condition and clears you for participation. (x-rays, MRI’s, operative reports, physician notes).  It is important that you know the date of injuries, dates and descriptions of surgeries, etc.  If the physicians are unsure of any of your history, they will require notes from your physician.  This may delay your participation in Truman athletics.  
  • Concussions, Loss of Consciousness, or Head/Neck Injury
  • Fractures or Dislocations
  • Severe Muscle Strains or Ligament Sprains
  • Surgeries that may affect athletic participation
  • Allergies or Allergic Reactions
  • Family History of Heart Disease or other disorder (High BP, Diabetes, etc)
  • Conditions or disorders involving major organ systems
  • Cancer
  •  Heart condition (bring physician clearance and documentation of diagnostic testing)
  • Asthma

The Athletic Training Staff at Truman wants to make athletic participation as safe as possible for the athletes.  If, during your physical exam, our Team Physicians feel that you have a medical condition or injury that puts you at risk, you will not be allowed to participate until your safety can be assured.  This may require information from your physician, diagnostic testing, x-rays, EKGs, or a visit to a specialist.  Although this situation is rare, you must understand that since your condition is pre-existing and not a result of participation in Truman athletics, you will be responsible for any bills that result from this testing.  We will do our best to help you work with your insurance company to coordinate payment.
